Adding Location option to Submit coupon form and Modding Form

Hi All,

I am new to this but I am wanting to add a category to the submit coupon form that allows users to choose their location and mall name. Has anyone modded the coupon submit form to include other fields.

Also I would like to change the heading of the form from "submit your coupon" to "Submit your coupon or sale"?

Finally I have added an upload image plugin on the submit your coupon page so that people can upload their own images. However what I am looking for is a reference generator that can generate a random unique number that the person must enter include with their number upload and also add this to the submit file. That way I can link the two together in the backend. This is a temporary solution while I wait and hope that app themes add this upload image feature to an update. Any ideas or suggestions?
