    Custom Forms malfunction causes del of custom fields after upgrade to WP 4.1 & CP 3.4

    Dear,

    Custom Forms malfunction is causing deletion of custom fields. After creating the fields and saving the form, fields disappear.
    This issue started happening after upgrade to WP 4.1 & CP 3.4.
