How To Install Screenshots not showing & Transaction is Undecided & Pending

The screenshots in the help thread on How To Install -
http://forums.appthemes.com/ccbill/how-install-57527/ are not showing.
It looks like I already have all bases covered from reading the Installation Instructions.
However, my tests transactions are going all undecided and pending - [Payments Undecided and Pending].
Is there a way to automatically approve the payments first and get the listing reviewed for approval later or is this done in one sweep in Order Actions. There seems to be 3 Order Actions - [Mark As Completed], [Mark As Failed], [Reset Gateway]. what do all these Order Actions mean and when do I invoke which action.
Is there more documentation or instructional video on how this payment gateway works?
