How do I make map work?

I want the map to show the trip mileage information / view from google maps based upon the zip codes gathered on a posting form...
I am currently creating the form to replace the one used when posting. It will require the zip code of where the equipment / load is available at and if its a load the zip code of were its going as well.
How do I link this information to google maps and show in sidebar?
