Custom Form Conditional Input

Apologies if this is already possible, or if it's available in a plugin, if so I haven't figured it out yet..

I'd like to add a custom field which is a 'Yes' or 'No'... But, if 'No', a 'please provide details' box appears allowing the user to add info.

Basically, 'Yes' results in 'Yes' being displayed, where 'No' turns the option into a text input.
