Multiple categories in Nav Bar

Hello,

Someone asked this question in another post, along with a second question, and only got the second question answered...

In my menu bar, instead of it showing CATEGORIES, I would like to seperate categories so that Menu Bar will show MAKE MODEL PRICE FUEL EFFICENCY and then have categories within does displayed. Basically same layout as it is now, just the ability to subdivide the category menu in the navigation bar. I normally would have used the MENU in appearance but the categories do not show up there, so is there another way or is it custom coding again?
