Url modification with State or City at the end?

Hi, I have proved some plugins to generate url with "cp_state" and "cp_city" in the url, at the end.

I don't win how to create these URL on my site, is there any plugin to make it?

For example:

From:

http://mydomain.com/ads/blackberry

to:

http://mydomain.com/ads/blackberry-Madrid

Custom Field Madrid on Url permalink.
