Customizing the Registration Form

We purchased the $159.00 version of Vantage but now I have questions. We like the registration form but we would like to have two different versions of it. First of all, we'd like to have a free listing that only includes Title, Address, Phone Number, Map, and a link to their Facebook page, allowing them to put their business in two categories. We don't want any other parts of the current form available for free listings. Then we want to use the current form for our featured listings.

Is there a way to do this? If so, how do we do it? If not, how can we separate the free listings from the featured ones. If everyone can use the current form, even free listings, what do we have that's extra to offer our featured listings? I realize there are other threads on this topic and I've read them but didn't find a satisfactory answer.
