Form Builder - possible to add form to ALL categories and it appear before title?

I have checked out the form builder tutorial and it doesn't do quite what I want...

I would like to add a simple two option radio button at the beginning of the create listing form (before title) and it should apply to ALL categories.

In other words if I add new categories later this form edit should apply to those too - ideally without having to edit the form and ideally without having to check each category of which there are many when I build the form.

What is the best way to do this please?
