I notice on the demo it doesn't matter which country you choose the states are all US

Is there an option once logged in to configure the County and related State options?

I see there are threads for this but useless if I'm not a registered user as I can't read them

By default, the "State" field has the US States included, however these field values can be easily deleted/replaced with locations relevant to your Country from within the "Edit custom fields" screen as shown here in the ClassiPress admin demo. You can also change the "State" title to something else relevant to your location. Thanks.

Does that mean though you can't set up a list of states that appear in the drop down if US is chosen (as in the demo) and a list of counties that appear if UK is chosen. It seems you are saying you can only have one countries states so regardless of which country you choose you will only ever see the field values set in the custom fields section. Have I misunderstood?

You have understood correctly, there is not a separate dynamic drop-down for every country available. One solution to this could be a separate instance of ClassiPress per country where you would select the relevant country from the homepage and proceed from there. This would involve the use of a WordPress multisite network and additional customization though. Thanks.
