All fields have the same value even though different values were entered

I filled out the form for an add and wrote 2,000 in one of the fields and different values in each of the other fields. When I completed the form, published and previewed it all of the fields use the number 2,000 and not the other info that I entered.

I had the same issue with the radio. I had 3 different fields with a radio checkbox but it only allowed me to make one choice for all three fields (as if the fields were connected but they were not). I turned them into checkboxes and it solved that problem but I guess the fact that the 2,000 repeats itself means that all fields are seen as one.
