    Editing Custom Form

I created a custom form, but the there are way to many fields which make the form too long and might scare people off.

I need to make the optional fields hidden either by a accordion style or some sort of wordpress plug in but I can't even start becuase I have no idea where to go to access the form that I have created.

Where does the new form go to? Straight to the database or where please help me
