Broken Login and Register pages

I made a few changes to the theme and somehow broke the Login and Register pages in the process. Not only is the styling slightly broken, but some of the functionality and content is also missing such as the password strength checker and the actual register button.

Here's what's happening:

1) http://www.mydomain.com/login/ = a broken page (see attached image)
2) http://www.mydomain.com/register/ = a broken page (see attached image)

But if I use the following URLs in place of the above:

1) http://www.mydomain.com/login/?redir...Fnav-menus.php
2) http://www.mydomain.com/register/?re...Fnav-menus.php

Can anyone explain what's happening here?
