Custom forms issue

I have created a custom form and applied it to all listing categories, but in the front-end when a category is selected it does not appear.

Example:

Cat 1, Cat 2, Cat 3 are my categories. In the front-end when I select Cat 1 custom forms don't show, however when i select Cat 2 or Cat 3 or any other categories custom form appears. I tried to deactivate all plugins and it did not resolve the issue.

I am assuming this is a bug that needs to be fixed.

Also, do custom forms override each other?
