Create listing & using dropdown option for categories doesn't trigger custom form

I've set up a custom form to be used when I submit a listing to a specific category.

When using checkboxes for the category it works fine, and I can see the new form.
However, when using 'dropdown' for the category selection the new custom form doesn't appear and trying to submit the listing results in an error.
