I think its a great area you're dealing with being the medical field, and the customization on the single ad listings is JUST what I was looking for!!
Care to share the how to on this one? I'm talking about this ad listing right here, the way you displayed the custom fields :

I think you could remove the Ad Ref ID and the whole comments section without too much trouble. It shouldn't be too tricky to make the drop down boxes wider too perhaps?

How do you put another template on the "front" page?
