Vantage 4.0 Forms

Thanks. I need unique forms for each category. I tried the Form below the Vantage wordpress Icon with one Category and is fine. If I need another category does not work. If I use the generic WP forms the fields are mixed with the Vantage form. Any advice? Some knows if somehow Advance Custom Forms may be compatible and what needs to be done?
