3.0.5.1 and Pie Register?

Is there something special I need to do, I can't seem to get the Pie Register fields to show up on the registration page. I need to collect Name and Website upon registration.

I'm starting with a clean install at the moment, but I'll be upgrading 3 sites with this same setup very soon.

Ok, making progress, the fields are present, but not visible.

I've set them as required. I attempted registration, but get an error because required fields are missing- first name, last name, image validation.

Ok, this seems to be the needed line

do_action('register_form');

I just don't know exactly where to put it. When I inserted that in theme-login.php, I ended up with the extra fields above the header bar. I believe it belongs somewhere around line 192, but I'm a php newbie so I'm not entirely sure.

Maybe not correct, but I did get it working

add <?php do_action('register_form'); ?> to line 48

The above does put the extra fields on the registration form, however, the custom emails that pie-register/register-plus allows are not being sent.
