I'm assuming there's a stylesheet not being called for other pages on my site, once I figure that out, I'll post it here.

Oops!

Remove the width declaration from #custom_search_form so you're left with:

#custom_search_form {
margin: 0 auto;

The form will then centrally align without having to be 'pushed' to the middle
