How can 'fixed text' instruction-fields be added to custom form

Would like to know how, if possible, to add some instructional text to a custom form. The tool-tips are good, but I'd like to have info 'up close and personal' and in their face in case the miss the tool-tip.

I've tried creating a text-area field, then going to the database and inserting text there, but the field still comes up clear/empty.
