edit code for 'post an ad' form

am looking to edit the 'post an ad' when someone uploads an ad.

am looking for somethig like this:

Select all [] if checked yes, if not 'no'

+ Category 1 (toggle)
[] a
[] a
[] a
[] a
[] a
[] a
[] a
[] a

+ Category 2 (toggle)
[] a
[] a
[] a
[] a
[] a
[] a
[] a
[] a

etc...

when checked it appears in the ad display.
I understand if the form builder doesnt do this...
but would you know where in editor i can customise the code?
