Forms and Custom form not upadating

i have tried all i can to create form but after creating the form it shows empty field. even when i try to create the custom form. after creating form and clicking on create it returns an empty page. i have re-installed the theme more than three times, i have removed all plugins and re-installed also gone to your forum only to find out that many users are having this same issue with no precise solution. Please help because am stork and cant continue with the job.
