Unable to customize fields or create form layouts

Hi,

I am unable to create custom fields or create form layouts. It give me an error message saying that the initial script did not install properly and to reactivate my theme, which I did several times and nothing happened. I checked with my host and it is not on their end.

My host said that this is a quick fix if my support could login as me (the admin) and go look at the script as it is a minor error.

I really need this resolved as my users can not post ads with info such as a description, price, location, etc.
