Custom form to PDF

I want the entire page to convert to PDF along with the data entered through custom form.

This plugin was able to convert only the main area and not custom form details- https://wordpress.org/plugins/wp-advanced-pdf/

When I discussed it with the plugin developer, he asked me to get details about the technicalities of the custom form from the theme developer so that he can work upon.

I tried to look to this page-

Can you tell the technicalities on how custom form works so that I can explain to plugin developer and he can modify the plugin accordingly.
