Cannot upload/create resume

Hi,
I just bought the Jobroller theme and have installed it. I am in the process of testing - and my first test to try and create (and upload) resume failed. It gives me an error -

1. ERROR: Please fill in all required fields. I have filled each and every field multiple times but it continues to give me the same error.

2. Also, the Resume Location field, even though it is filled with genuine city, the google map does not load and gives an error - Oops! Something went wrong.

3. I am surprised to see that there is a upload for resume photo! and not a resume in word format! I even entered a jpg image there - but cannot test it because the errors mentioned above keep coming back.

Are these known issues? Does anyone have a solution for this?

Where do I send this for support?
