Adding Custom Forms to Add Listing Screen?

I've created a new custom form with two text field. However it doesn't appear in my add listing screen. Am I missing something here?

I basically need to add additional data to the listing screen to accommodate things like firstname / lastname and a few other fields. Any help appreciated.

I am familiar with child themes and using the functions file etc...
