custom form bugs
there are several bugs in the custom form
1- cannot have multiple custom form assign to same category
2- old ones do not refresh the list of categories
after creating a custom form you cannot add it to new categories created after
3- the title of the text field do not have any html tag = no css (view attachment customformhtml)
and it does save all the time the title of the field
