    Custom Forms fields disappeared from front end

    On my website Custom Forms fields disappeared from some listings from front-end.
    Data exists on backend and database but not visible on front end.
    After wandering through forums I found need to upgrade vantage.
    So, my vantage version is latest one that is 3.0.1 but after upgrade issue still persists.

    Please provide solutions for the same.
