Custom forms won't show when creating a listing

I've created a custom form just like in the video:

I added the categories, but when I go to 'create listing', the form doesn't show when I choose the category. So what happens in the video at 6:00 doesn't work for me.
(It's also not visible in the backend of a listing, but I don't know or thats normal).


The form is created but it just doens't work. Some things what it's not:
- There are no plugins installed
- I used no symbols in the custom fields.
- The permalink structure i use is /%postname%/
- I've set up Vantage just link the guide and all the rest works.

is there some kind of setting I missed?
