Error Message when trying to set up Custom Form

I am attempting to Set Up details about each listing using Custom Forms.
I have been able to set ONE checkbox and it functions...but when I attempt to add more than one....I get an error.
I have tried trashing the form and starting over...but that doesn't work....I get the same error message.
Attempting to set up as Checkboxes for: Delivery Y N, Take Out Y N, Dine In Y N, Reservations Y N......then additional info about the listing.
But only get so far.(one)

The error message reads:
