    File Uploads in Custom Form Not Working

    I have a custom form with two file upload fields. I can't upload using the feature though. When I upload it opens the Backend Wordpress Media Uploader, uploads and processes the file, but the INSERT INTO POST button isn't highlighted and won't allow you to click it.
