whatts the possibility of making the site multicity and International.

This can definitely be done, and has successfully been done by a number of ClassiPress customers..

How would you set this up? Do you install multiple instances of Wordpress/Classipress, or is this with Wordpress MU?

I really depends on if you want each instance to share the same database. I think in the case of that example, they may have just installed instances of CP on different subdirectories, but didn't share databases as he didn't need the users to be able to post across all of the different cities. It is definitely possible though, and the Ultimate Edition ClassiPress licence is MU enabled.
