Can't stop spam user registrations

I set up my site some time ago and neglected it somewhat as I moved onto other things. Nothing much happened with my site as I never actively promoted it. However, more recently I have had a raft of spam registrations and attempts to post spam ads. Ads need moderator approval so they don't appear so that's not an issue but I thought that I had a reCaptcha system to only permit 'real' users to register.

So, as practically all the traffic is spamming, I unchecked the box that allows anybody to register. Unfortunately, I am still getting 30-40 registrations per day. How???

I have WP 3.4.2 and ClassiPress 3.0.5.3.

So, my questions are:

1. How are spammers getting past the reCaptcha?
2. Even with registration disabled, they are still registering
3. I thought that there was a setting somewhere that I used to require all users to have moderator approval before being allowed to post but I can't readily find that option - have I imagined that?
4. What do I do to get my site back under control as I now want to promote it for genuine use?
