Issue with additional information after update
Hello,

The addtional information ( added fields for custom form ) that gets added to the form when people fill it out gets cramped up really bad and looks like crap. Can you please give me the proper code to clean this up or come up with a fix for this.

See image below. I blotted out all clients info for privacy concerns.
