    Only One Checkbox or Radio Button Displayed for an Entire List of Options

    I am using the personal edition of 3.0.5.3.

    I have created a handful of custom fields that are to either display an array of checkbox options to the user or a drop down list.

    When I view the form on the live site, the options merge together so that only one option is available.

    In the forum, I've read posts from others with related problems, but not what I am seeing. I also looked at this bug list: http://forums.appthemes.com/bugs-list-v3-0-5-a-15813/

    Has anyone else seen this problem?
