Pictures get uploaded with missing Title attribute after upgrade to WP 3.5
After update to Wordpress 3.5 I have noticed two things:

1. Pictures get uploaded to blog posts with missing Title attribute.
2. Hitting Edit image pops up an old window from previous Wordpress version (see attachment 1) instead the latest 3.5 (see attachment 2).

Any advice how to fix this?
