Upgraded to Clipper 2.0.1 - Unable to type Coupon Code

I recently updated clipper theme to the latest version, the problem is that when I go to add coupon and select coupon type 'coupon code' there is no text box to write the coupon code into.

I have read all the solutions available on your support forum, they primarily involve going to Forms and assigning coupon type to coupon code ---> but here is the problem I have no forms defined on my forms page.

I tried to manually add the form and then set it to coupon code, this way the text field shows and I am able to enter the code however it never appears on the website.

Please resolve at your earliest as this website is the primary source of my revenue.
