Custom form

I'm a bit confused as to how to set up the listing form the way I want it.

1) The default form, is it possible to just change that from the admin? or do I have to go into the php and change it there?

2) If if create a custom form, how does that become the basic/default form or replace the basic/default form?

Right now I'm unsure if I want to change the default form or create a whole new one, as the default form is fairly close to what I want anyway.
