Custom form confusion

I don't get how custom forms are used or where they appear.

Does a custom form replace the default "add a business" form?

Or does it get added to it somehow?

Where does the custom form appear?

I only need 1 type of form, but it's a bit different from the default "add a business" form that is currently there, so how do I set that up? modify the default form through the PHP/HTML or I can completely replace it (but keeping most of the default forms options anyway) ?

Because I only need 1 type of form I don't want it attached only to ONE category, it needs to be for all categories, how do I do that?
