How to add a custom form - it's not working? Vantage theme

Hi,
Vantage theme:
I have tried adding a custom form and have ticked a category and it's just not showing up when i go to create a new listing.
Can someone explain exactly how to add another field to the listing form? I want to add an email address field (why this wasn't a standard field i'll never know).

I'm just stuck with phone, twitter, website, facebook contact fields

Would also be handy if the email address was clickable, somehow building in a mailto link to whatever the customer types as their email address.

Any help would be appreciated.
