How to add new 'Checkboxes' and 'Radio' to forms. Set data to default selection?

Hello.

I'd like to add a few new selections to the business form. I have successfully added the 'Checkboxes' and 'Radio' area in the forms. I also see it when viewing a listing.

My problem comes when importing the data. I would like the default selected to be 'Unknown' (as pictured in the attachment). I added and imported the data with the columns looking like the other attached pic.

Please help me solve this.
