Custom Form dependant on Pricing Plan chosen

I am having troubles with this setup. Mainly because I have a somewhat complicated pricing structure. I have 4 different pricing plans with different offerings available for each (nothing to do with featured, just a few more listing options like a long description and website address) - I need JUST certain items to show up on the form fields dependent on what pricing plan they choose. How can I assign custom forms in this way? So NO OTHER form fields show up?? I don't even know where I can edit the generic form that is in there right now. Would be nice is that was editable. I don't know PHP and assumed this theme was a bit more user friendly. Logically you could take your custom form (which I figured out how to do) and assign it to the price plan. Is this not doable?
