Adsense showing PSA (public service ads) on front page

Hi All,
I've just encountered miss adsense showing as PSA not actual ads on above header at front page and also missing adsense on sidebar front page.
I've tried everything such as review TOS, adding privacy policy and so on.

any body can help to solve it? I'm afraid google will deactivate my account if the PSA still shown on front page.
