The CATEGORIES list in the MENU doesn't build consistently.
My site has 39 top-level categories (no sub-categories). When clicking the CATEGORIES popup link from within the navigation menu, it builds the lists in columns of 10 for the first two rows, then everything else is in the third row.
So for example, row 1 has 10 categories, row 2 has 10 categories, and row 3 has 19 categories.
My settings in the CP Admin > Menu Category Options are -- # of Columns: 4, # of Sub-Categories: Show All
This is not an issue with the Home Page "Ad Categories" Menu. That menu properly displays 3 colums with 13 categories in each column.
