Form fields don't clear after adding store

When adding a new store through the WP Clipper admin area, upon saving the store the "name" and "slug" fields clear but the remaining fields on the page all remain with the store data. The issue then is that when you add the next store you need to be sure and overwrite or blank out each field so that it doesn't retain the information from the previous store.
