HELP! Custom form not displaying in Add Listing

I created a custom form using custom form builder cause vantage doesn't permit anyone to add fields to existing basic form.

I added 2 listings in WP admin but custom form is NOT included in listing. I did watch http://docs.appthemes.com/tutorials/...-form-builder/

On one of the listings, the custom form displayed but then it disappeared when I added address for the map. I bought vantage less than 2 weeks ago.

WHY isn't custom form fields NOT appearing in Listing > Add New ???
