Creating form and new tab

Hi,
I'm looking to create a custom form and then would like to be able to take the information from that for and add it to a tab between the "Overview" and "Reviews" tab.
I've found the ID that's created by the form in the database, but when I try to add a tab and post the data, everything is skewed and the new tab is not clickable.

Has anyone done something similar??

Anyone have any suggestions or documentation??
