registration page first_name

Hello,

My registration page is out of format - don't remember adding the columns - First_Name, Last_Name and display name - now the format of the fileds are off -

Can any one help me which is the file that needs to be edited.

I did look at the code in tpl-registration.php - couldn't find anything related to first_name/last_name or displayname.

Pl. help.
